Complete the Narrative Report Requires Adobe Reader 7.0 or higher. Download and install the free Adobe Acrobat Reader. (Reader 7.0 includes spell checking) Best practice suggests that you prepare your narrative in MS Word and save it for your files. Then, copy/paste from your document into the online form. In addition, partially completed forms may be saved on your computer (see How does this work?), but creating a MS Word document remains the best practice. Note: Some users may receive a message that "Adobe Reader does not save the data that you enter into form fields." Ignore this message and click OK to proceed -- you will be able to save your partially completed form by clicking the "Save Form" button on the last page of the pdf form and following the instructions. If your report is successfully submitted, you should see a pop-up message and you should receive an automated email message with a copy of the report attached. |
