Survey Templates
Library Mad Libs
Make gathering customer feedback fun with this take on Mad Libs that asks users to fill in the blank about their library visit.

These surveys were made available by the Library Research Service and you can find more information about survey design and administration their website. Templates will download in Microsoft word and can be modified to meet your library's needs.
Tips for Communicating Data:

Use a combination of stories and data to appeal to the heart and mind of readers for maximum impact.

Put data in context. Use ratios or per capita to breakdown large numbers and make them meaningful.

Include library and community information. Incorporating demographic data can help demonstrate why what the library provides is important to community well-being.

Remember to let the reader know how to get involved. Provide contact information or ways to help support the library's work.
